Report of Continuing Disability Interview - Form SSA-454-BK is used to collect information from individuals receiving disability benefits or their representatives. The information is evaluated by SSA to determine whether the individuals remain eligible for benefit payments because they continue to be unable to engage in substantial gainful activity by reason of their impairments. Information is obtained concerning sources of medical treatment, participation in vocational rehabilitation programs (if any), the opinion of beneficiaries regarding whether their conditions have improved so that they are able to return to work and attempts to return to work (if any).
